WSIB Form 26 – Health Professional’s Progress Report

IMPORTANT! This feature is no longer available in Universal Office due to WSIB requirement to have it submitted strictly online. However, we strongly recommend to download the completed form and attach it to the documents tab of the patient’s case. For more information see Attach and Manage Attached Documents.

To perform the following procedure open the Patient Manager.

  1. Select patient, and then select WSIB case from the list.
  2. Click on the Documents tab
  3. Click [New Document] from the toolbar.
  4. In the Template Type field, select Form-26 from the drop-down list, and click Open.
    The Health Professional’s Progress Report (Form 26) appears on screen.

Note: Many fields in this form are already pre-populated with information already entered in the patient’s profile.

  1. Fill in the fields for each section of the form. Use the section links on the left to advance through each page.
  2. In Part 4 and Part 5, there are multiple pages to complete. Click Next>> located in the lower right corner of the form to view and complete additional pages.
  3. Once you have completed the form, click Preview to preview the Form 26 official document.
  4. In the Preview window, click Print to print the document. Close the window to return to the previous screen.

Click Save and Close to save your changes and to return to the previous window, or click Cancel to exit without saving.
